How to get hold of the on call provider:

If the office is closed, please call (541) 567-6434. If this is a non-emergency, you may leave a message with the answering service. In the event of an emergency, the answering service will contact the on call physician. The on call physician will then return your call.

Our patient portal gives patients the ability to access their health information, message their care teams, request appointments, and much more. Please contact our office to get started with your patient portal or sign up at your next visit!

If you do not see an appointment available that works for you in our online scheduling system, please call our office and ask for a Patient Experience Coordinator.

Unfortunately, at this time we cannot schedule Immigration physicals or DOT physicals via our online scheduling system, please call our office to schedule these appointment types.
